How to look up an NPI
You can search three ways. Enter a 10-digit NPI number directly, or search by last name plus state (first name is optional), or search by organization name plus state. The number search is exact; the name searches return matching records in the file.
The result card shows the provider's name, NPI, record type, taxonomy, practice address and practice phone, enumeration date, last update date, and a staleness flag when the record has not been updated in a long time. If the record has a deactivation or reactivation date, that is shown too. When other NPIs share the same practice address, they are listed on the card.
NPI format and Type 1 versus Type 2
An NPI is 10 digits. The tenth digit is a Luhn check digit, computed with the prefix 80840, which is the CMS method. If a number fails that check, it is not a valid NPI.
Type 1 means an individual provider. Type 2 means an organization, such as a group practice or facility. The record type is shown on every result card.
How current the file is
This is not a live CMS registry query. It is the NPPES monthly dissemination from September 2026, with records through 13 September 2026 and the deactivation report of 14 September 2026. Weekly files after 13 September 2026 are not in this copy. From 13 September 2025 through 13 September 2026, 37,540 records have a deactivation date, and 36,070 of those are still deactivated. Separately, 2,251 records have a reactivation date in that window. That count is not the remainder of the 37,540.
A provider corrects an NPPES record at the CMS NPPES site (nppes.cms.hhs.gov), not on this page. Practice phone and practice address are the values in the dissemination file. They are not a verified direct dial, and they are not an email. Treat every result as a starting point and verify details before relying on them.
NPI versus other identifiers
An NPI, a DEA number, a state license number, and a taxonomy code are different identifiers that answer different questions. This tool searches NPI only. Taxonomy is shown on the result because NPPES stores it on the NPI record; it is not a license number.
