Chiropractic taxonomy codes: NPI counts by code and state
The general chiropractor code (111N00000X) dominates the registry: 98,209 active individual NPIs list it in at least one of their taxonomy slots, and 96,375 list it as their primary taxonomy. Every specialty code is small by comparison; the largest, Sports Physician (111NS0005X), covers 2,789 NPIs. This page breaks down the twelve chiropractic taxonomy codes by how many active individual NPIs carry them, separates primary-taxonomy counts from any-slot counts, and shows how chiropractors with a chiropractic primary taxonomy are distributed across the fifteen most-listed practice states. All figures come from the CMS NPPES monthly file (September 2026 release, records to 13 September 2026), aggregated by Provyx (CMS NPPES download page, observed 13 September 2026).

Contribution: first-party data, derived analysis, downloadable dataset
- Collected: counts from the CMS NPPES September 2026 file, where each active NPI contributes one row per distinct taxonomy code in its 15 taxonomy slots (aggregated 13 September 2026), and the 12 codes of the NUCC Chiropractor classification, code set 26.1 (retrieved 2 October 2026). Taxonomy codes are self-reported by providers.
- Published: 53 NPI counts, each covering at least 10 NPI records (smaller groups are withheld), and 1 code-set figure (the 12 codes of the NUCC Chiropractor classification).
- Method: the exact queries, row counts and thresholds are in the methodology.
- Computed: 16 derived figures (top-3 and top-5 totals, state shares and remaining counts) from the published figures. Each is a dataset row with its formula, inputs and expression, so it can be recomputed.
- Tracked: in our source-fact ledger since 4 October 2026; no changes recorded yet.

The distribution is top-heavy. The general code alone accounts for 98,209 NPIs, and the top three codes combined (general, Sports Physician and Rehabilitation) reach 103,472 NPIs (Provyx aggregation of the CMS NPPES file, observed 13 September 2026). Adding Nutrition and Orthopedic brings the top five to 106,137. The seven codes outside that top five, from Neurology down to Thermography, together cover only 3,289 NPIs. Thermography is the clear outlier at 92, just over a quarter of Radiology's 335 and a rounding error next to the general code.

The primary-taxonomy set tells the same story with slightly smaller numbers, and the gap between the two sets shows how often specialties appear as secondary codes. Sports Physician drops from 2,789 any-slot NPIs to 2,102 primary, Rehabilitation from 2,474 to 1,774, and Nutrition from 1,499 to 1,017. The top three primary codes combined cover 100,251 NPIs and the top five 102,187, while the seven codes outside the top five cover 2,158. For most specialty codes, the NPIs that list the code also name it as their primary taxonomy: 2,102 of 2,789 for Sports Physician and 1,774 of 2,474 for Rehabilitation. Pediatric Chiropractor is the exception: it sits seventh by any-slot count (511) but tenth by primary count (240), so more than half the NPIs listing it carry another code as primary. Internist and Occupational Health also move up the ranking, from ninth to seventh and from tenth to ninth.

Fifteen states are listed, each with its share of all 104,345 active individual NPIs with a chiropractic primary taxonomy (the total includes Puerto Rico, other territories and a few records with a non-US practice address). California leads at 12,849 NPIs, 12.3% of that total, followed by Texas (7,760, 7.4%) and Florida (7,539, 7.2%). The top three states combined hold 28,148 NPIs and the top five (adding New York and Illinois) 38,070. The ten states outside the top five, from Pennsylvania down to Colorado, together account for 31,908 NPIs, each between 2.5% and 4.1%. Unlike the taxonomy codes, the state spread is gradual: no single state approaches the general code's dominance, and the middle of the table is tightly packed, with Georgia, Washington, New Jersey and Michigan all at 3.1% to 3.4%.
What this means for teams buying or building provider data
Three decisions follow from the figures.
Segment by specialty code, but expect small pools. If your targeting depends on a specialty such as Sports Physician (2,789 any-slot, 2,102 primary) or Rehabilitation (2,474 any-slot, 1,774 primary), the addressable universe is in the hundreds to low thousands, not tens of thousands. The seven codes outside the top five total 3,289 NPIs across all specialties combined. Campaigns built around a niche chiropractic specialty need multi-state scope from the start; a single-state slice of, say, Radiology (335 NPIs nationally) will be very thin.
Weight state coverage toward California, Texas and Florida, but don't stop there. Those three states hold 28,148 of the chiropractors with a chiropractic primary taxonomy, and the top five 38,070. Because the remaining ten listed states still hold 31,908, a coverage plan limited to the biggest three misses more chiropractors than it reaches.
Treat the general code as the base layer, and decide how to count specialties. With 96,375 of the 104,345 chiropractic-primary NPIs naming 111N00000X, the general code is the primary taxonomy for more than nine in ten chiropractors in the registry. Most NPIs that list a specialty name it as their primary taxonomy, but many list it in a secondary slot (687 of 2,789 for Sports Physician; 271 of 511 for Pediatric Chiropractor), so a filter on "primary taxonomy = specialty" captures a smaller group than a filter on "lists the specialty anywhere."

Limits of this data
Several caveats bound what these counts can tell you.
Snapshot, not trend. Every figure is a single observation of the NPPES monthly file released September 2026, with records to 13 September 2026. Nothing here measures growth, churn or seasonal movement, and no record in this comparison states a change over time.
One provider can appear under several codes. The method behind these counts is that each active NPI contributes one row per distinct taxonomy code in its 15 taxonomy slots. The any-slot set therefore double-counts providers who list two chiropractic codes, which is why the primary-taxonomy set is the cleaner measure of how many chiropractors identify with each code. The two sets should never be added together.
Active registration is not practice volume. An active individual NPI means the provider is registered in NPPES, not how many patients they see, how many hours they practise, or whether they currently treat patients at all. The counts measure registry presence. "Active" means the NPI is not deactivated in the September 2026 file.
Codes are self-reported. Providers choose their own taxonomy codes in NPPES, and every count here is a number of NPI records, not of people or state licences. A specialty code shows what the provider selected; the registry has no field that confirms a specialty credential.
Organization NPIs are a separate population. Besides the individual counts above, 62,127 active organization NPIs (NPI-2) carry a chiropractic primary taxonomy (Provyx aggregation, observed 13 September 2026). Group practices and clinics sit in that set, not in the individual figures.
Coverage is partial by construction. The state comparison includes the fifteen states listed; 36 of 51 jurisdictions (50 states and DC) are not included in this comparison. All twelve codes in the NUCC Chiropractor classification clear the minimum group size of 10 NPIs, so both taxonomy tables list every chiropractic code. The counts cover chiropractic taxonomy codes only, not the full provider market.
For a current, reproducible count, the underlying NPPES files are publicly downloadable from CMS, so any figure on this page can be checked against the source registry.
How to use the dataset
The CSV and JSON files hold 70 rows: one row per aggregate group (each published figure), plus one row per derived figure with its formula and inputs (54 observed, 16 derived). The rows behind each group (the collected source rows) are not published.

Each observed row's source_url is its public source (the page the data came from) or, for a figure built from several sources, the section of the source manifest that lists them; derived rows list their input rows in scope.
To use it, filter attribute to the figure you need, sort by number, and cite the row's source_owner and observed_at. Recompute any derived row from its expression and the input rows its scope lists.
