What organization NPI update dates can tell you

In these 13 retained organization profiles, 6,982 of 27,965 NPI records have a last-update date before October 1, 2021. That is an age-of-record-change measure anchored to October 1, 2026. Use it to group records for field-level verification.

Our data

We computed this report from 27,965 NPI records in 13 existing legal-organization profiles. The downloadable dataset contains 13 rows at the legal organization level. The cohort consists of these selected legal-name profiles; each retains its own source boundaries.

Variation is large within this selected cohort

CVS Pharmacy Inc. has 1,139 older-date records out of 1,234, or 92.30%. National Vision Inc. has 909 out of 1,283, or 70.85%. Luxottica of America Inc. has 1,213 out of 1,855, or 65.39%. Each share uses the exact legal-name extract in the table as its denominator.

At the other end, Novant Health Medical Group LLC has no records before the cutoff among its 546 rows. Publix Super Markets Inc. has seven among 1,167. The gap creates different review priorities; explaining it requires evidence about how each extract was maintained. An administrative update process, a change affecting many records, or the composition of the selected legal-name group could affect the distribution.

Keep the registry timestamp and verification evidence separate

A date describes when something was recorded as changed. Field-level checks require a separate evidence trail. Labeling a record “verified” requires a recorded check of the relevant field. The same problem occurs if every older record is labeled “bad.”

Keep update age as one signal beside field-specific evidence. A delivery workflow might separately record when a practice website confirmed an address or when a business phone was checked. Those observations need their own source and date. Store them alongside the NPPES update date, which remains useful for understanding the underlying registry version.

Record-weighted and organization-weighted questions differ

Walgreen Co contributes 11,140 records, with 1,689 before the cutoff. That is the largest absolute older-date count in the cohort even though its percentage is lower than several other profiles. A verification team allocating effort by record volume would see a different priority order than a team screening organizations by their share of older dates.

Choose the question before ranking the rows. For a workload estimate, compare absolute counts. For the composition of one organization's extract, compare its percentage. For the full selected cohort, compute 6,982 divided by 27,965 rather than taking the average of organization percentages. Both calculations describe this selected cohort.

A practical way to create a verification sample

Use the age groups to stratify a sample, then inspect actual fields in each group. Include both older and more recent records, several legal names, and different states. Comparing both age groups lets a reviewer test whether update age helps distinguish records needing correction from those that remain useful.

Define a field-level outcome before checking the sample. “Address confirmed on a current practice website” is an observable result if the website and date are retained. “Record looks stale” is an interpretation that different reviewers may apply differently. Report confirmed, contradicted and unresolved outcomes separately. The aggregate age counts provide a sampling frame for that separate verification task.

The cutoff is fixed to the retained source analysis

The source organization pages used October 1, 2026 as their age anchor. This report preserves that choice and uses a strict earlier-than comparison against October 1, 2021. A record updated exactly on the cutoff date is outside the older-than group. The CSV makes the numerator and denominator visible so the arithmetic can be reproduced.

Using a new current date without labeling the change would gradually move records between groups even if the source file never changed. For a monthly refresh, publish both the source vintage and the analysis anchor. If the cutoff moves, distinguish that calendar effect from actual NPPES updates. Comparing the underlying source rows and the cutoff identifies those two effects separately.

Evidence to gather during verification

During verification, record address deliverability, phone response, operating status and ownership from current sources. An organization’s registry-review process requires its own evidence. Retain the source and observation date for each conclusion so another reviewer can trace the decision.

Use these percentages to plan which source records to inspect. The cohort was assembled from existing public profiles, with each legal name retained as a separate group. Its value is in showing the range of registry update-age distributions and giving an analyst a reproducible starting point for a more specific verification task.

Retain both the source date and the evidence trail

When enriching an account database, keep the source-system timestamp, the file vintage and any later verification timestamp in separate fields. Collapsing them into one “last updated” column makes it difficult to explain what changed. An enrichment-run timestamp records when the process ran; field timestamps record the age of the evidence.

The downloadable comparison provides counts, the fixed cutoff and the missing-date count for each legal name. Follow a profile link to inspect its retained records, then record any newer evidence as an additional layer. That approach preserves a reversible history without pretending that this aggregate analysis completed the verification.

Method and reproducibility

Read last_update_date from every retained organization CSV. Count a record in the older-date group when its date is nonempty and earlier than 2021-10-01. Divide by the total NPI rows in that legal-name file. Retain the original profile age anchor of 2026-10-01, rather than moving the cutoff to the date this report was generated. Missing update dates are counted separately; none occur in this cohort.

Limitations: The date reflects the NPPES record, not a separate verification date for each phone, address, specialty or ownership field. A recently changed record may still contain old fields, while an old record may remain accurate. These selected legal names are not representative of all organization NPIs. The analysis cannot establish deliverability, phone response, opening status, current ownership, registry-review frequency or correction rates. It does not explain differences between organizations or rank their operations and commercial data suppliers. Those conclusions require field-specific observations and a suitable comparison design.

This analysis and its prose were prepared with AI assistance. The 13 input CSV files are the downloads already attached to the organization profiles below. We read identifiers and ZIP codes as text, preserve legal names, and calculate each organization independently. The provenance file records every input SHA256 and row count. The analysis code is included with the reviewable source; the public CSV and JSON contain identical rows. The derived downloads contain aggregate organization-level counts and source-field summaries.

Underlying source: CMS NPPES dissemination files, identified in each profile as the September 2026 monthly release. The public CMS index was under maintenance when this analysis was prepared; we used the retained extracts and their recorded provenance, using the original recorded source vintage. CMS explains the NPI standard.

Source profiles and state drill-downs

Each profile links to its existing state page and filtered source records. Those pages provide the next level of detail; use them to inspect the records behind a legal-name distribution. Start with CVS Pharmacy Inc. in Texas, Novant Health Medical Group in North Carolina, or the organization research directory.
