Dental email list: how many US dental providers are in NPPES

Anyone shopping for a dental email list eventually asks the same question: how many dental providers are there to reach? The best public answer comes from the CMS National Plan and Provider Enumeration System (NPPES), the registry behind every US NPI. New to the registry? Start with NPPES: Definition and Guide. Provyx aggregated the September 2026 NPPES release (records to 13 September 2026). It counted 294,428 active individual NPIs whose primary taxonomy sits in the Dental Providers grouping (Provyx aggregation of the CMS NPPES monthly file, observed 13 September 2026). This page breaks that total down by classification, taxonomy code, practice state and how recently each record was updated, then looks at what the numbers mean for anyone building or buying provider data.

Our data

Contribution: first-party data, derived analysis, downloadable dataset

  • Collected: counts from the CMS NPPES September 2026 file, where each active NPI in the Dental Providers taxonomy grouping is counted once under its primary taxonomy (the slot flagged primary, else slot 1), aggregated 13 September 2026. Taxonomy codes are self-reported by providers.
  • Published: 45 NPI counts, each covering at least 10 NPI records (smaller groups are withheld).
  • Method: the exact queries, row counts and thresholds are in the methodology.
  • Computed: 42 derived figures (top-3 and top-5 totals, group totals, shares, what the listed rows leave out and remaining counts) from the published figures. Each is a dataset row with its formula, inputs and expression, so it can be recomputed.
  • Tracked: in our source-fact ledger since 5 October 2026; no changes recorded yet.
  • Sources: the source manifest (JSON) lists the source behind the figures (registry files), each with its public URL, retrieval date, status and the reason any is left out. A card's or table's Source link goes to that source's own page.

What the dental provider classifications show

Dentists dominate the Dental Providers grouping. 274,573 of the 294,428 active individual dental-provider NPIs (93.3%) carry a Dentist primary taxonomy, with Dental Hygienists next at 16,175 (5.5%) and Dental Assistants at 2,866 (1.0%) (Provyx aggregation of the CMS NPPES monthly file, observed 13 September 2026). The top three classifications together account for 293,614 NPIs, a 99.7% share of the total. The remaining four classifications are much smaller: Denturists (454), Dental Therapists (206), Advanced Practice Dental Therapists (86) and Dental Laboratory Technicians (68).

The concentration matters for list building: a dental email list built from NPPES primary taxonomies is overwhelmingly a dentist list. If your campaign targets hygienists (16,175) or assistants (2,866), the registry is a thin base: an NPI is required mainly of providers who bill health plans, so many hygienists and assistants never apply for one. The four smallest classifications hold between 68 and 454 NPIs each.

Which dentist taxonomy codes hold the most NPIs

Within the 274,573 active individual dentist NPIs, two codes dominate. General Practice (1223G0001X) accounts for 126,633 NPIs (46.1%) and "Dentist, no specialization" (122300000X) for 94,649 (34.5%) (Provyx aggregation of the CMS NPPES monthly file, observed 13 September 2026). The top three codes, adding Orthodontics and Dentofacial Orthopedics (13,722, 5.0%), cover 235,004 NPIs, an 85.6% share. The top five, adding Pediatric Dentistry (11,041, 4.0%) and Oral and Maxillofacial Surgery (8,555, 3.1%), cover 254,600 NPIs, or 92.7%. The nine remaining specialty codes together hold 19,973 NPIs, 7.3% of dentist NPIs.

Two practical points follow. First, a list filtered only on General Practice (1223G0001X) would miss the 94,649 dentists (34.5%) whose primary code is "Dentist, no specialization" (122300000X). Second, the specialist segments are small in absolute terms: Endodontics (6,755), Periodontics (6,689) and Prosthodontics (3,949) are the largest of the nine, and Oral Medicine has just 50 NPIs. Specialty-targeted dental campaigns work from a much smaller pool than the headline dentist count suggests.

Where dentists practise: the state spread

The table lists the 15 practice states with the most active individual dentist NPIs; each share is of all 274,573, which include records outside the 50 states and DC. California leads with 39,655 dentist NPIs (14.4%), followed by Texas (21,055, 7.7%) and New York (18,002, 6.6%) (Provyx aggregation of the CMS NPPES monthly file, observed 13 September 2026). The top three states hold 78,712 NPIs, a 28.7% share; the top five, adding Florida (16,751) and Illinois (11,446), hold 106,909, or 38.9%. The 15 listed states combined account for 186,402 NPIs, leaving 88,171 NPIs (32.1%) outside them: in the other 36 of the 51 jurisdictions (50 states and DC), in Puerto Rico and other US territories, at military (APO/FPO) addresses, or with a non-US or blank practice location.

The middle of the table is tightly packed: Michigan (8,781), New Jersey (8,775), Massachusetts (8,626), Ohio (8,334) and North Carolina (7,740) sit within about a thousand NPIs of each other. The ten states outside the top five together hold 79,493 NPIs (29.0%), so a national campaign cannot be planned around five states alone; 61.1% of dentist NPIs (167,664) fall outside the top five.

How stale are dentist NPI records

Of the 274,573 active individual dentist NPI records, 153,026 (55.7%) were last updated 10 years or more before 1 October 2026, and another 38,222 (13.9%) five to ten years before (Provyx aggregation of the CMS NPPES monthly file, observed 13 September 2026). That puts 191,248 records, a 69.7% share, at five or more years since their last update. Only 83,325 records (30.3%) were updated within five years, including 27,582 (10.0%) within the last year.

Update age is one of the few freshness signals the registry carries, and a list built without filtering on it will lean heavily on records the provider has not touched in years. For record staleness across all provider types, see the NPI Staleness Report.

What this means for dental email list decisions

A few conclusions rest on the figures above:

  • Segment by taxonomy, not just "dentist". General Practice holds 46.1% of dentist NPIs and "no specialization" 34.5%, so a filter on either code alone drops more than half of dentist NPIs. Build segments across both broad codes before adding specialties.
  • Treat specialty campaigns as niche. The nine codes outside the top five hold 19,973 NPIs (7.3%). Orthodontics (13,722) and pediatric dentistry (11,041) are the only specialties with five-figure pools.
  • Plan state coverage beyond the biggest five. The top five states hold 38.9% of dentist NPIs; the ten next-largest hold 29.0%. A list covering only California, Texas, New York, Florida and Illinois misses more dentists than it reaches.
  • Filter on update age and say so. With 69.7% of dentist records last updated five or more years ago, ask any list vendor how they handle stale NPPES records, and whether they verify phone or email against a second source.
  • Individual versus organization matters for outreach. 274,573 of the 385,355 active NPIs with a Dentist primary taxonomy are individuals (NPI-1, 71.3%) and 110,782 are organizations (NPI-2, 28.7%); the Type 1 vs Type 2 NPI distinction explains the difference. If you are emailing practitioners, you want the individual records; organization NPIs point at practices, not people.

Limits of this data

Several caveats bound what these counts can tell you.

  • Coverage. The figures cover active NPIs in the CMS NPPES September 2026 release whose primary taxonomy falls in the Dental Providers grouping. "Active" means the NPI has not been deactivated; it does not show that the provider is licensed or practising today. The counts are NPI records, not people, licences or practices, and they do not cover every practising dental professional: many hygienists and assistants never apply for an NPI. They say nothing about email addresses, phone numbers or any other contact detail; NPPES is a registry, not a contact list.
  • Self-reported taxonomy. Primary taxonomy is the slot the provider flagged (or slot 1 otherwise), labelled with NUCC taxonomy 26.1 (see Healthcare Taxonomy Code: Definition and Guide). Providers self-report these codes, so misclassification is possible and the counts inherit that.
  • Snapshot, not trend. All figures come from one file release observed on 13 September 2026. Nothing here describes growth, churn or change over time.
  • What a record's unit measures. Each unit is one NPI, not one person and not one practice location. A dentist with two NPIs (individual and organization) appears in both entity types, and the state counts reflect the recorded practice location, which may differ from where the provider actually works. Practice locations with a non-US country code or no state are counted as "non-US or blank"; Puerto Rico, other US territories and military (APO/FPO) codes are kept as their own groups.
  • Update age is not accuracy. The bands measure time since the record's last update relative to 1 October 2026. A never-updated record (last-update date equal to the enumeration date) may be perfectly current; a recently updated one may still hold an old address.

How to use the dataset

The CSV and JSON files hold 87 rows: one row per aggregate group (each published figure), plus one row per derived figure with its formula and inputs (45 observed, 42 derived). The rows behind each group (the collected source rows) are not published.

Columns: id (row id (observed rows r01..., derived figures d01...)), kind (observed or derived), entity, attribute, scenario, value, number (the figure as a plain number), unit, scope (the scope, or a derived figure's input row ids), input_price_scope, scenario_scope, formula (a derived figure's formula in words), expression (its arithmetic over the input figures), source_owner, source_url, observed_at, fixture.

Each observed row's source_url is its public source (the page the data came from) or, for a figure built from several sources, the section of the source manifest that lists them; derived rows list their input rows in scope.

To use it, filter attribute to the figure you need, sort by number, and cite the row's source_owner and observed_at. Recompute any derived row from its expression and the input rows its scope lists.
