New NPI registrations hit an all-time record in 2025, up 48% since 2020. The growth is not where most people expect: behavior technicians, counselors, and nurses drive it, and one profession now out-registers every physician specialty combined.

A Decade of New Provider Registrations
Every US provider who bills insurance needs an NPI, and the enumeration date on each record marks when they entered the system. Counting registrations by year gives a clean, public measure of how fast the healthcare workforce pipeline is growing, and in what shape.

Registrations grew in nine of the last ten years, and the growth is accelerating: the decade's largest single-year jump came in 2025, up 83,265 over 2024. (2026 shows 243,697 registrations through the August file cut; we exclude partial years from every claim.)
The Boom Is Behavioral
Break the 2025 class down by profession and the story stops being about doctors. The largest single group of new providers in America is behavior technicians, the front-line workers who deliver applied behavior analysis, mostly to children with autism. 114,857 of them registered in 2025, more than every physician specialty combined, and 234% more than in 2020. Payer credentialing rules that require technicians to carry their own NPIs drive part of the count, and the underlying industry growth drives the rest.

Two more shifts stand out. Registered nurse registrations more than doubled since 2020, reflecting RNs increasingly carrying their own NPIs as they move into roles that bill directly. And the counselor and social worker lines confirm what the behavioral segment shows from the other side: the behavioral health workforce is the fastest-expanding part of US healthcare, on every measure this file offers.
A note on what is absent from the table: physician specialties. Physicians typically receive an NPI during training under a student classification and reclassify after residency, which makes recent-year physician counts systematically understated. We exclude them rather than publish a misleading trend. Residency match data is the right source for physician pipeline questions.
Why New Providers Are the Highest-Intent Segment in Healthcare
For anyone selling into healthcare, a newly enumerated provider is the most valuable record in the registry. A new practice chooses an EHR, a practice management system, billing support, suppliers, insurance panels, and marketing inside its first year, and most of those decisions never reopen until something breaks. The vendor who arrives during that window sells into an open decision; everyone after sells against an incumbent.
The registrations data says that window opens 631,277 times a year and rising. Filtering the registry by enumeration date is free (631,277 records is small enough to work), and pairing recent enumeration with specialty and geography turns it into a targeting strategy. One caution from the companion study: the same registry that records new providers so well maintains existing ones poorly. Per The NPI Staleness Report, 46.1% of active records have never been updated after registration. Use enumeration dates for discovery, and verify everything else against live sources.
Methodology
Source: CMS NPPES full data dissemination file, August 2026 vintage, joined to the NUCC Health Care Provider Taxonomy reference. Each record's Provider Enumeration Date determines its registration year; profession uses the provider's current primary classification (Type 1 records only). 2026 is excluded everywhere as a partial year. The profession table omits the student classification (a transitional status) and all physician specialties, for the understatement reason described above. State-level cuts exist in the underlying dataset and show expected population ordering (California, Florida, Texas, New York lead), with occasional anomalies that reflect organizational registration behavior rather than workforce location; we publish state figures on request rather than as findings. All figures recompute when we ingest a new NPPES release.
